The AMCB exam is the gate between a nurse-midwifery program and practice as a CNM. It is a 175-question, four-hour exam across the full scope of midwifery care. Here is the exam, a realistic study plan, the fee, and how recertification works.
| Exam | AMCB national certification examination (CNM) |
| Format | 175 multiple-choice questions (includes unscored pretest items) |
| Time | 4 hours |
| Passing standard | Criterion-referenced; pass/fail set on the whole test, scored on correct answers only (no penalty for guessing) |
| Recent pass rate | 80% first-time (2024); 51% repeat |
| Initial exam fee | $425 (per the AMCB fee schedule); re-examination $350 |
| Recertification | Certificate Maintenance Program, 5-year cycle (3 maintenance modules + 20 contact hours) |
Exam specifications and fees change, so always confirm current details with AMCB before registering.
The CNM credential is earned by passing the national certification examination administered by the American Midwifery Certification Board (AMCB) after completing an ACME-accredited graduate nurse-midwifery program. The exam is criterion-referenced and covers antepartum and primary care; intrapartum, postpartum, and newborn care; and gynecologic, sexual, reproductive, and primary care. Most candidates test soon after graduating. This guide covers the exam, an efficient study plan, eligibility, the fee, and the Certificate Maintenance Program that keeps the credential current.
CNM certification is the gateway to practicing and billing as a nurse-midwife, not an optional pay differential. The income change comes from moving into the CNM role after a graduate program, not from the certificate itself. Compensation depends on setting (hospital, birth center, private practice), call burden, region, and state practice authority. Earning the credential without stepping into a midwifery role does not raise an RN's pay.

AMCB certifies both the Certified Nurse-Midwife (CNM, for nurse-midwives) and the Certified Midwife (CM, for non-nurse midwives) through the same examination. The CNM credential requires an active RN license; the CM does not. Both lead to AMCB certification, but state licensure recognition differs, so confirm what your state board recognizes.
AMCB certification is valid for five years. All certificants are auto-enrolled in the Certificate Maintenance Program (CMP), which requires completing three Certificate Maintenance Modules (one in each practice area) plus 20 contact hours of approved continuing education (the modules do not count toward the 20), and an annual program fee due each May 31. Letting the five-year cycle lapse forfeits the certificate. Confirm current requirements with AMCB.
Certification renewal is one of up to four clocks an APRN keeps current, alongside the RN license, the state APRN license, and DEA registration. See the NP & APRN renewal hub for how they fit together by state.
