The NCC NNP-BC exam is the gate between a neonatal nurse practitioner program and practice in the NICU as an NNP. It is a 175-question, three-hour exam focused on the neonate from resuscitation through follow-up. Here is the exam, a realistic plan, and how recertification works.
| Exam | NCC Neonatal Nurse Practitioner core board certification (NNP-BC) |
| Format | 175 questions (150 scored, 25 unscored pretest) |
| Time | 3 hours |
| Passing standard | Criterion-referenced; an ability level set against a predetermined standard, no fixed percentage |
| Recent pass rate | ~89% in 2024 (NCC publishes a combined all-takers rate, not a first-time-only figure) |
| Initial exam fee | $325 (includes a non-refundable $50 application fee) |
| Recertification | Every 3 years via the Continuing Competency Assessment (CCA), which generates an individualized education plan |
Exam specifications and fees change, so always confirm current details with NCC before registering.
The NNP-BC credential is earned by passing the Neonatal Nurse Practitioner core board certification examination administered by the National Certification Corporation (NCC) after completing an accredited graduate NNP program. The exam is criterion-referenced, with no fixed percentage to pass, and covers the full scope of neonatal advanced practice: the general assessment and management of the neonate, the disease processes by system, and professional issues. This guide covers the exam, an efficient study plan, eligibility, the fee, and the three-year maintenance program.
NNP-BC certification is the gateway to practicing and billing as a neonatal nurse practitioner, not an optional pay differential. The income change comes from moving from the bedside into the NNP role after a graduate program, not from the certificate itself. Compensation depends on the unit, call and night burden, region, and employer. Earning the credential without stepping into an NNP position does not raise an RN's pay.

There is one route to NNP board certification: the NCC NNP-BC exam. NCC no longer accepts certificate-prepared (non-graduate) applicants, so a graduate NNP degree is required. Do not confuse the NNP-BC with NCC subspecialty certifications (such as neonatal intensive care RNC-NIC), which are different credentials for different roles.
NCC certification is valid for three years. Maintenance starts with the Continuing Competency Assessment (CCA), which produces an individualized education plan specifying how many CE hours you need and in which areas; only CE earned after the assessment, addressing the plan, counts. A maintenance application and fee are also required. There is no single flat CE-hour number for active NNP-BC maintenance, it is plan-driven. Confirm the current requirements with NCC.
Certification renewal is one of up to four clocks an APRN keeps current, alongside the RN license, the state APRN license, and DEA registration. See the NP & APRN renewal hub for how they fit together by state.
